Abstract

Hypertension is a chronic disease with an increasing prevalence that requires lifelong management. In the working area of Meninting Primary Health Care, West Lombok Regency, the incidence of hypertension is alarmingly high, reaching 2,929 patients in 2024. This high prevalence is exacerbated by low public knowledge, medication non-adherence, and suboptimal family involvement in patient care. This community service program aims to enhance the independence of hypertension patients through the GARSIH program (Reduce Salt and Healthy Lifestyle Movement) using a Family-Centered Care approach. The program was conducted in Pelempat Hamlet through three main stages: initial health screening, health education regarding low-sodium diets, and intensive self-management mentoring that positions the family as the primary companion (medication supervisor and diet regulator). The results of the activity indicated a significant increase in the community's health literacy and the establishment of family commitment to supporting patients' lifestyle modifications. In conclusion, structured family-based mentoring through the GARSIH movement is an effective promotive and preventive strategy to optimize self-management and control blood pressure among hypertensive patients in the community setting.
